For workers’ comp claims, civil litigation, mediation, reserve setting for insurance companies, discharge planning, Medicare set-asides, and elder care planning: evaluation and planning for complex future medical costs and care needs.  Development of a Certified Life Care Plan starts with utilizing Nurse-CLCP standardized procedures for gathering information including: interview with client and family members, reviewing medical records and supportive documentation such as depositions, day in life videos, school records, employment records and tax returns, consulting with treatment providers and/or experts, and researching future costs and sources for treatment.  All of this information is then taken and turned into a very detailed and comprehensive Life Care Planning report.

Services Offered:

• Life Care Planning/Assessment Report: A comprehensive detailed report outlining future medical costs and care needs for clients affected by catastrophic injury or disease. Topics considered for inclusion in report include: evaluations, therapies, diagnostic testing, medical and adaptive equipment, aids for independent functioning, prescription and non-prescription medications, home/facility care, routine medical care, transportation, architectural modifications, potential complications, surgical intervention, and vocational services.
