Ongoing Registered Nurse Care Management (RNCM)

For Any Persons overwhelmed with medical care management for themselves or a loved one, especially persons with complex medical and/or psychosocial needs; 24/7 concierge nurse care management, “nurse on speed dial” always available to help guide you through the most difficult health-related storms and provide advocacy as needed ensuring the best possible outcomes for the client and peace of mind for their loved ones.

Services Offered:

  • One-Time Detailed Assessment Report with Recommendations: RNCM will conduct a one-time in-person visit to the client’s home for nurse evaluation of both the patient and their living environment. RNCM will conduct a thorough medical review of client by obtaining a health history from client and/or family, medical record review, and in-home medication review.  RNCM’s health history review will include review of all relevant labs, diagnostics, doctor visit notes, and any hospital records.  RNCM will assess patient’s cognitive abilities and nutritional status using nationally standardized tools.  RNCM will then solidify all of this information into one detailed assessment report (usually about 12 pages) that will conclude with actionable recommendations tailored specifically for the client.  (Cost: $800/individual, $1,200/couple– includes 30-mile one-way radius from Salida/95368, additional charges for mileage extending past this)

  • Ongoing Nurse Care Management: RNCM works collaboratively with client and family to build patient chart, obtain medical history, review medications, identify/anticipate needs, provide advocacy, and function as a liaison between client/family and care providers to ensure the best possible medical outcomes. RNCM provides highly customized ongoing recommendations/advocacy/interventions using a holistic “whole person” nursing approach that takes into consideration of the client’s wants, needs, and overall well-being all within the backdrop of their spiritual and cultural beliefs, family dynamics, and resources available.

    • Standby Only: Minimal level of service, for clients whose care is fully managed by themselves or their loved ones who want additional piece of mind knowing RNCM can step in at any time with hands-on nurse care management/support during time of crisis or in the event that family are not available. **Requires initial One-Time Detailed Assessment Report
    • Supplemental: Moderate level of service, for clients whose care is partially managed by themselves or loved one(s) that want on-going regular hands-on assistance with managing the care of the client where specific tasks are delegated to RNCM as needed. **Requires initial One-Time Detailed Assessment Report.
    • Comprehensive: Extensive level of service, for clients whose care is minimally managed by themselves or loved ones, or for clients/loved ones who want to delegate the lion’s share of care management to the RNCM; Working collaboratively with the client/family, RNCM would attend all doctor’s visits, conduct regular online record review, manage/set up appointments, mange care-giving support, and manage medications, just to name a few. **Requires One-Time Detailed Assessment Report—this can be waived during crisis if comprehensive care management services are initiated immediately in which case a “good faith” deposit will be required and applied to the first month’s invoice.


***All ongoing RNCM services during normal business hours are invoiced monthly via billable hours at a rate of $75/hr for travel time, and $145/hr for all other billable services. For emergencies that take place outside of normal business hours, “After hours” services/billings are available at rate of $250/hr including travel time.
